Upon receipt of your request and form, our payment provider Fibonatix, will email you a link to make the payment for the Day 2 & Day 8 testing kits. Once we have received your payment, we will email you your unique reference identification number, which you need to provide on your passenger locator form. Please note that your arrival date to the UK will be classified as Day ‘0’. Under the Day 2 and Day 8 program, you will have to take your sample on day 2 and day 8 of your self-isolation. For example, if you arrive in the UK on Monday 10th April 2021, (Monday 10th April will be classified as Day ‘0’), you will take you Day 2 sample on Wednesday 12th April 2021(This will be Day 2 after your arrival date) and Day 8 sample on Tuesday 18th April 2021(This will be Day 8 after your arrival date).

BRONZE SERVICE: By Mail: We will post you 2 testing kits separately. Day 2 kit will be posted before Day Zero (Day Zero is your arrival day into the UK). Day 8 testing kit will be posted to you after Day 4 of your arrival in the UK.
SILVER SERVICE: IN CLINIC: if you want to give your sample in our clinic. Please book an appointment for both day 2 & day 8 by completing the form below. Please remember to bring your passport.
